You may be eligible to receive an 18.9% rebate on your electricity bill effective May 1st, 2021 under the Ontario Rebate for Electricity Consumers Act, 2016. If your account meets any of the eligibility criteria below, please check off all boxes that apply, and sign and date the form. Once completed, please return to PUC Services.
